Bladder exstrophy is a rare condition in which a baby is born with the bladder open on the lower part of the abdomen, along with differences in the abdominal wall, pelvic bones and genitals. Epispadias is the milder end of the same spectrum, where the urinary passage opens on the top of the penis. These are among the most complex conditions in paediatric urology and need specialised reconstructive surgery.
Treatment is staged reconstruction — closing the bladder and abdominal wall, repairing the genitals, and working towards urinary continence — followed by lifelong follow-up of the kidneys, bladder and continence. With expert reconstruction and careful follow-up, children can achieve good function.

मूत्राशय एक्सस्ट्रोफी एक दुर्लभ स्थिति है जिसमें शिशु का मूत्राशय पेट के निचले हिस्से पर खुला होता है, साथ ही पेट की दीवार, श्रोणि की हड्डियों एवं जननांग में भिन्नता होती है। एपिस्पेडियास इसी समूह का हल्का रूप है, जिसमें मूत्र-मार्ग लिंग के ऊपरी हिस्से पर खुलता है। ये बाल मूत्र-रोग की सबसे जटिल स्थितियों में से हैं और इनके लिए विशेषज्ञ पुनर्निर्माण सर्जरी आवश्यक होती है। इलाज चरणबद्ध पुनर्निर्माण है — मूत्राशय एवं पेट की दीवार बंद करना, जननांग की मरम्मत, एवं मूत्र पर नियंत्रण की दिशा में काम — इसके बाद गुर्दे, मूत्राशय एवं कंटिनेंस की आजीवन निगरानी।
